Collecting student data is a great way to prepare for your classes with intention. Knowing who your students are, their names, where they're from, and their intentions for your class can make a difference in how you go about instructing them.
The Google Form below is a simple way to collect information quickly and efficiently (for the teacher and student), and to analyze and synthesize that information. This is a First Day Survey to get a glimpse at who my students are, and what they'd like for me to know about them. It gives me an understanding of their experience with the subject matter, their interest in the class and subject overall, and some insight into what their interests might be, and how I can include that in my lesson planning.

It allows for easy access for students to complete and submit quickly. This helps for absent-minded students who might have difficulty remembering to complete the assignment!
It allows for quick and organized information for the teacher. We have a lot to do, so to gather information from potentially hundreds of students and review it in a matter of minutes is a big time-saver!
Google Forms is fairly customizable. Teachers can add different types of question formats, and allow students to upload images, links, videos in their responses.
Google Forms can also be used for quizzes, quick student polls, RSVPs for family events, and more!
This assignment uses the SAMR Model, by Augmenting this lesson using technology. It takes what might normally be a written assignment, and alters it to allow for better functionality for students and teachers.
